Forms are to be completed online only. After completion you will receive an email copy of your form. Please print, sign, and mail with payment. We must receive your registration forms and payment before the cut-off dates to ensure discount rates.
WE DO NOT ACCEPT FAXED CONVENTION REGISTRATION FORMS.

Assistant Teachers/Senior Students (13 & Up)
Includes free admission to competition. Assistant Teachers/Senior Students are allowed in designated ballroom only. Eligible to audition for scholarships.

Junior Students (7 & Up)
Includes free admission to competition. Junior Students are allowed in designated ballroom only. Eligible to audition for scholarships.

Teachers/Studio Owners
Includes ALL-ACCESS to both ballrooms, a teacher/parent forum with faculty, reserved seats and free admission to the competition.

Teacher Discount
Bring 10-25 students ......(1 Teacher FREE).
Bring 26-39 students...... (2 Teachers FREE).
Bring 40+ students .........(3 Teachers FREE).

Observers
Allowed with paid students only. Observer (NO teachers, dancers, studio owners, or managers) may observe all classes in both ballrooms. Includes competition ticket.

Multi-City Discount
Receive a 10% discount on convention fees for the second city you attend.
Receive a 20% discount on convention fees for the third city you attend.
CONVENTION FEES ONLY. NOT APPLICABLE TO FINALS.


FORMS OF PAYMENT
We accept Personal Checks, Business Checks, Cashier Checks and Money Orders. We also accept credit card and bank account payments through PAYPAL for your convenience. Upon receipt of your CONVENTION REGISTRATION FORMS, we will email you an invoice and instructions on how to use PAYPAL.

IMPORTANT

• Fees for U.S. cities are in U.S. currency.

• Fees for CANADIAN cities are in Canadian currency and include G.S.T.

• Please, no babies in any ballroom.

• CHAPERONES are required for all dancers 15 and under who are staying at the hotel.

• All dancers must wear cover ups when leaving ballroom area.

• We ask everyone's cooperation in respecting the privacy of other hotel guests and all hotel property.

• CONFIRMATION will be emailed to all entrants upon receipt and process of your registration. Please present your confirmation to the pre-registration desk for admittance for you, your staff, students, and observers. Registration at 7:30 AM, Saturday, in every city. Confirmation will also be emailed to single registrations (not connected with any particular school).

• Registrations are NON-TRANSFERABLE, and NON-REFUNDABLE. Registrations will be limited. Registrations received 21 days or less prior to convention may not be acknowledged with a confirmation. Pre-registration is strongly advised otherwise registration on first morning of the convention cannot be guaranteed.

• NO VIDEO CAMERAS allowed at convention. Flash photos allowed during classes only.

• Extreme Dance Management reserves the right to refuse admittance, or have removed from the convention anyone who is disruptive, unruly, or displays rude conduct. To be determined at management's sole discretion.
